A common foot problem that we see in my office is heel pain and/or
heel spur syndrome. This problem has the hallmark symptom of "pain
after rest". Especially the first step of the day (after sleeping) is the
most painful step. A combination of X-rays and sonogram may be used
to determine if a heel spur is present. The spurs may vary in size. Often
times a small spur may be more painful that a large spur! But large
spurs can also hurt quite a bit! Most spurs are also accompanied by
inflammation of the plantar fascia which is a ligament that is located in
close proximity to the spur. If a spur is not present, the pain is usually
due to the localized inflammation of the plantar fascia ligament.
There are many ways to treat heel pain, some of them are oral anti-inflammatory drugs, orthotic (inlay)
therapy, corticosteroid injection(s) and of the plantar fascia. Corticosteroid injections are given directly to
the painful area allowing concentrated healing to take place. In some instances a surgical release of the
plantar fascia ligament and removal of the spur may be necessary. Successful heel pain treatment usually
requires patients to be persistent and to try a few different treatment or combination therapies.

Another name for painful contracted toes is hammertoes. This
common problem is seen in our office very often. Hammertoes are
caused by a tendon imbalance in the feet. The feet have two sets of
tendons, the extensors on the top of the feet and the flexors on the
bottom. When one set of tendons overpulls or pulls too hard the result
is hammertoes. This is a genetic problem that can be accelerated by
wearing pointy high heeled shoes. Surgical treatment is usually
necessary once the problem has progressed to contract the digits.
The minor surgery is done in the office and involves removing a tiny piece of bone from the affected
toes. This is done through a very small incision on the toe and usually requires only a few stitches. Most
patients return to work on Monday following a Friday surgery (rest over the weekend). After about 10
days the stitches are removed and you are on your way! Usually a sneaker or comfortable shoe is the
first to fit comfortably and then you gradually get back into your dress shoes. After the procedure, most
patients find that their feet feel much more comfortable while walking in their normal shoes.





Orthotics are inlays that are inserted into your shoes. They
are custom made from an imprint/mold of your feet and fit
your feet perfectly. The mold takes only about 30 seconds
and is made very accurately. They are made in a similar
technique to shoemakers fabricating custom made shoes.
The inlays feel great and can/may help a variety of foot
problems from painful bunions and hammertoes to arch pain,
heel pain, and ball of foot problems.
The molds are then sent to be fabricated in professional laboratory and is specifically made just for
your feet. Turn around time to patient is about two to three weeks. After an initial break-in period
of about two weeks, most patients feel more comfortable and most feel improvement immediately!
Orthotics are great for people who walk or stand all day because of the excellent perfectly
matching support to the underside of your feet. They also are great for athletes and also commuters
who walk on harsh concrete to and from work!

Neuromas are benign enlargements of digital (toe) nerves. They are
commonly caused by entrapment resulting from compressive forces
especially against a ligament in the foot. Most commonly patients
complain of a burning sensation "like walking on a hot pebble".
Pain is aggravated by walking in tight (high heel) shoes. Patients
often describe an overwhelming desire to remove the shoe, rub the
forefoot and flex the toes. This usually provides dramatic (although
short) relief.
Evaluation involves X-rays and sonogram with treatment of a combination of orthotic devices and
corticosteroid injections. Surgical removal is sometimes necessary and involves removing the painful
